Regional Happenings

We have developed a map showing which NYS counties are involved in planning activities to address the housing, transportation, and service needs of the aging population. Additionally, you can view a map showing which NYS counties have already begun to implement some of these services. You can see that most of the state is already empowering communities to prepare for aging in the community on our combined planning and implementation map.

Following the November 2008 Empowering Communities for Successful Aging Conference, NYSOFA hosted conference calls with those individuals who identified themselves as willing to be leaders/liaisons and those willing to host a training in their region. The calls took place on 2/24/09 and 2/26/09, and there were approximately 50 people on each call representing regions across the state. You can review a summary of the conference calls.

In order to organize our ongoing work we divided the state into regions. Regional Map Use these links for each region to find out what is happening in your community:

• Manhattan -
  • Answering the Call: A Community Forum on Healthy Aging and Civic Engagement - June 12, 2009
  • Grand Street Settlement NYSOFA grantee - This planning process will encourage older adults living in the Lower East Side to articulate needs and create a vision for how they wish to age in place.
  • Heights and Hill Community Council NYSOFA grantee - This project, which will feature a needs assessment and research about short and long term needs, had as its impetus a burgeoning partnership among a small group of older community members in Park Slope interested in developing more livable communities.
  • Isabella Geriatric Center NYSOFA grantee - This planning project, entitled the Upper Manhattan Age and Community Initiative, targets communities that are racially and ethnically diverse and will build on the work of the Upper Manhattan Partnership for Senior Independence, a partnership of eight community agencies providing case management service for older persons.
• Brooklyn
  • Claire Heureuse Community Center, Inc. NYSOFA grantee - This proposed planning process will assess the unmet needs of the aging population in this community with a focus on the immigrant aged residing in the target area who face language and cultural barriers when attempting to access services.

Long Island:  Nassau, Suffolk

• Nassau
  • Town of North Hempstead Department of Services for the Aging NYSOFA grantee - Following a comprehensive planning process, the Town proposes the establishment of the Senior Home Assistance Registry Project (SHARP).
• Suffolk
  • North Fork Women for Women Fund, Inc. NYSOFA Grantee - This planning process, which targets the lesbian community in Riverhead and Southhold, will include four key steps:

Mid-Hudson Region:  Dutchess, Sullivan, Orange, Ulster, Putnam, Westchester, Rockland

• Rockland County
  • Empowerment initiative underway in Rockland County
  • Mental Health Association of Rockland County NYSOFA grantee - This endeavor builds on the "Project Tomorrow", a three-year public/private planning and program development initiative which identified eight key goals for aging in place.
• Sullivan County
  • Cornell Cooperative Extension of Sullivan County NYSOFA grantee - The proposed project will build on the collaborations that have resulted from the formation of the Sullivan County TRIAD, a group established last year to prevent elder abuse/senior violence and promote an improve quality of life for older adults.
• Ulster County
  • Family of Woodstock, Inc. NYOSFA grantee - The impetus for this project comes from a newly formed group, Staying in Place, comprised of skilled, experienced older adults who initially participated in the Senior Recreation Program established in Woodstock.
• Westchester County
  • Livable Communties conference. October 2.

Capital Region:  Albany, Saratoga, Columbia, Schenectady, Greene, Warren, Rensselaer, Washington

• Albany County
  • Albany County Department of Social Services NYSOFA grantee serving Albany and Schenectady county residents - The proposed project involves a two-pronged approach to planning for and creating aging and disabled friendly communities.

  • Upper Hudson Primary Care Consortium NYSOFA grantee – Serving Warren and Washington Counties - The Adirondack Rural Health Network will invite broad participation of consumers, service providers and community leaders in the planning process.

Mohawk Valley:  Fulton, Montgomery, Hamilton, Oneida, Herkimer, Schoharie

  • Mercy Care of the Adirondacks NYSOFA grantee – Serving Essex and Franklin Counties - A Steering Committee, composed of professional, community and consumer partners will guide a comprehensive needs assessment of four areas considered essential to creating an elder friendly community

Southern Tier:  Broome, Schuyler, Chemung, Steuben, Chenango/Tioga, Delaware, Otsego

  • Chemung County Department of Aging and Long Term Care NYSOFA grantee - This initiative will include collaborations between the Department, local and regional not-for-profit organizations, business groups, government entities, aging services organizations, educational institutions, community associations, faith-based organizations, older adults and other interested individuals.

  • Steuben County Office for the Aging NYSOFA grantee - An Aging in Place Leadership Team was recently formed in recognition that there are "naturally occurring retirement communities" in the County.

Western New York: Allegany, Erie, Cattaraugus, Niagara, Chautauqua

• Allegany County
  • Allegany/Western Steuben Rural Health Network, Inc. NYSOFA grantee -The Allegany/Western Steuben Rural Health Network facilitates the Allegany County Community Partnerships on Aging, a collaboration of health and human services organizations, governmental agencies, community and civic leaders, hospitals, older adults and other community members, that has carried out a comprehensive planning process to promote senior wellbeing, maximize independence, and improve the aging delivery system.
