• Transitions Network/Long Island Chapter - The Transition Network is a community of women over 50, who join forces as they navigate the transition from one career to another - or whatever is next. http://www.thetransitionnetwork.org



• Nassau
  • Town of North Hempstead Project Independence is a program designed to support Town residents 60 and over so they can remain as healthy and self-sufficient as possible in their own homes and communities. Town residents, their family members or caregivers can call 311 (within the Town, or (516) 869-6311 if outside the Town) with a Project Independence-related question or issue. Project Independence staff of social workers, nurses and community advocates will be in touch with a vast network of agencies, businesses and other institutions within the Town.
  • Town of North Hempstead Department of Services for the Aging NYSOFA grantee - Following a comprehensive planning process, the Town proposes the establishment of the Senior Home Assistance Registry Project (SHARP).
  • The following Naturally Occurring Retirement Communities are in place:
    • POB Cares NORC
    • Plainview Assisting Community Elders NORC
    • Town of North Hempstead, Project Independence NNORC.
• Suffolk
  • North Fork Women for Women Fund, Inc. NYSOFA Grantee - This planning process, which targets the lesbian community in Riverhead and Southhold, will include four key steps:
  • The following Naturally Occurring Retirement Community is in place:
    • Hands on Huntington NORC
