End of Life Issues

The Hospice Foundation of America(External Link) - http://www.hospicefoundation.org/ - exists to help those who cope personally or professionally with terminal illness, death and the process of grief and bereavement. You can find local resources and other useful end of life information at this site. This site's 'Caregiver's Corner' has caregiver tools, links to other resources and reading suggestions.

The Hospice and Palliative Care Association of New York State (HPCANYS)(External Link) - http://www.hpcanys.org/index.asp - is a not-for-profit organization representing hospice and palliative care programs, allied organizations and individuals that are interested in the development and growth of quality, comprehensive end-of-life services. Their website is designed to provide the public and their members with information about end-of-life care. You can find local resources such as a Hospice Program, a Palliative Care Program and a Hospice and Palliative Care for Children and other helpful information.

The National Hospice & Palliative Care Organization(External Link) - http://www.nhpco.org - their Helpline is 1-800-658-8898.

Caring Connections(External Link) - http://www.caringinfo.org/ - a program of the National Hospice and Palliative Care Organization (NHPCO), is a national consumer and community engagement initiative to improve care at the end of life, supported by a grant from The Robert Wood Johnson Foundation. This site is broken into the following sections: 'Are You Planning Ahead,' 'Are You Caring for Someone?,' 'Are You Living with an Illness?,' 'Are You Grieving a Loss?,' 'Are You Reaching Your Community?,' and 'Are You a Caring Business?'.

Hospice Net(External Link) - http://www.hospicenet.org/index.html - provides information and support to patients and families facing life-threatening illnesses. Hospice Net is an independent, nonprofit 501 (c) (3) organization, they do not accept advertising and is funded entirely by unsolicited private donations. This site provides a wide range of information for patients and families facing life-threatening illness, such how to find local services, what questions to ask, information for patients, children, caregivers and bereavement. Hospice Net's caregiver site(External Link) - http://www.hospicenet.org/html/caregivers.html - provides resources such as "What You Can Do To Be a Supportive Caregiver,' and 'Hard Choices for Loving People,' and much more.

The non-profit Aging with Dignity(External Link) - http://www.fivewishes.org/ - was established to provide practical information, advice and legal tools one needs to ensure your wishes and those of your loved ones will be respected. Their Five Wishes document helps you express how you want to be treated if you are seriously ill and unable to speak for yourself. It is unique among all other living will and health agent forms because it looks to all of a person's needs: medical, personal, emotional and spiritual. Five Wishes also encourages discussing your wishes with your family and physician.
